Isolation milling: Why are tracks disconnected from the pads?
Track and pad obviously seem to have different signals. So they belong to different nets. That's the simple reason why they are split. When designing a "PCB without schematic" it can happen, that the layout optically looks good but a connection between track and pad is not properly established.

When placing tracks please recognize always to hit the center of a pad. Take advantage of the snap-on boxes which appear at the end of the track when you come close to a pad (snap-on-pad). It pulls the track automatically to the center of the pad.

Menu item "Settings/Settings/Options" allows to display all signal names in the layout view. Zoomed out it might look a bit muddled but zooming in more and more it can be helpful having the names displayed.

Result: Only if pad and track have the same signal name (net) both belong together and will get milled the way that they have contact. Maybe it is necessary to delete the last segment of the track and connect it again properly in the way discribed.
